Registering with the Surgery                

New Patients                                                                                 

If you would like to register with the practice, please fill in a permanent registration form, available at reception, or you may download a copy Permanent Patient Registration (GMS1). When registering all patients will be asked to provide recent proof of address and complete a basic health questionnaire, available at reception, or you may download a copy New Patient Registration Form.  

Health Check for New Patients

Once registered you will be booked into an appointment with our HCA's who will do a height, weight and blood pressure check and will answer any queries you may have on how St David's operates. 

Temporary Residents

The Doctors are happy to see patients who are temporarily resident in the practice area should medical advice be required. In this instance persons wishing to be treated should state that they are a temporary resident when reporting to reception. To register you will need to complete our Temporary Resident Registration form which is available from reception, or may download a copy Temporary Resident Registration (GMS3).

Practice Boundary

St. David’s Clinic has an open list for people living within the practice area and operates a policy of non-discrimination on the grounds of race, gender, social class, age, religion, sexual orientation, appearance disability or medical condition.
