If you would like to register with the practice, please fill in a permanent registration form which is also available at reception or can be downloaded from the menu below. When registering all patients will be asked to provide recent proof of address and complete a basic health questionnaire, available at reception, or again you may download a copy below.

Please fill in both the "Family doctor services registration.pdf" AND the "New Patient Registration Form Sept 2023.Docx" as both are needed to register you.

IF ON REPEAT MEDICATION PLEASE PROVIDE YOUR REPEAT SLIP WITH THE REGISTRATION FORM IN ORDER TO AVOID DELAYS WITH MEDICATION ORDERING

St. David’s Clinic has an open list for people living within the practice area and operates a policy of non-discrimination on the grounds of race, gender, social class, age, religion, sexual orientation, appearance disability or medical condition.

If you are moving house, even if it is nearby, please notify us as soon as possible or you may be removed from our list by the Health Authority.  It will also enable us to update our computer records so that we can find you in an emergency.  Keeping us up to date with your telephone number is even more important.

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
